If you don't finish the mission before the date you simply don't complete it.
As far as refitting, you should be able to do it at any blue harbor I think. To be honest I've never refitted. U-boats returned to their home ports when they ran out of fuel or torpedoes, so when I run out I head home, get some rest and head out on a new patrol.
