The patrols still play out like in Sh3, so yes, you will find targets outside your assigned patrol areas.
Basically, during each campaign section (which covers a certain period of time) you're offered several missions. These are (often) large tasks that can last for several months and allow you to refit several times while you're on them.
An example would be "sink 100,000 tons of enemy shipping in the south-western approaches region".
Additionally, you sometimes get special missions like "recon that harbour" or "land a spy in the vicinity of...".
You can also combine missions, that is, work on one mission, then head for another area and try to fulfill your orders there.
So far I like the campaign in SH5, since it provides you with a more focussed experience that makes better use of the historical background.
It's nothing like the canned SH2 missions.
