When in your office, click on the radio that is on the left wall. That is where you access the options at. Select "gameplay options" and check the boxes that you want enabled. That will give you a custom realism setting.
Note: from my experience, setting the options from the "start screen" doesn't work. You have to use the radio in the office for your settings to apply to your campaign.
