This is a known bug "Base changed to Brest but does not show up on map yet".
Look at the nice map someone posted sail up that channel and exit patrol you should be close enought to get your renown.
As far as crew refusing to repair ... most likely rigged for silent running.
Yes I know your on the surface trudging along at flank... but the silent running button is probably still checked. Un check that and get to work sailor !
