Even if it was a re-fit no contractor in his right mind would do that installation it is a clear violation of the NEC.
But the quote "most dangerous thing Ive seen today" today is the right word. Ive seen just as bad if not worse things in my carrier.
Just recently I saw a hot tub installation where the owner had put a time clock in to run the hot tub certain hours. Not a bad idea really its where the time clock was put. It was hanging on the wall from a nail above the hot tub

wtih no GFI protection.
Ive seen circuit breakers held closed with a screw becuause and I quote "It kept tripping"
Ive seen 240V 'multi pole' loads served by two single pole breakers
Ive seen 40 ampere breakers with 20 ampere wire connected to them.
Not really dangerous but amusing and it did scare the crap out of us... Ive seen a 60" x 30" air duct get sucked flat like a straw when the smoke dampers closed and the air conditioner kept running

was standing right under it when it happed.
