One should clue the General into the fact that a hearing before the Senate is about as a formal a meeting as you can get. Use of full titles is appropriate if not mandatory.
It is not like the military does not spend a lot of effort smooching his butt and stroking his ego. Now it was his turn.

As to calling her mam it is military doctrin to address your 'superiors' as sir or mam. This much is clear.
She disrespected a man whom has probably worked 10 times harder than her and her limo jet liberal buddies.
