It is a very nice change. I started in the 29th out of La Spezia in 1942. You can start earlier out of Greece IIRC.
You see a lot more warships-I've seen a couple of cruiser task forces(no battleships or carriers yet),
a lot of single/double ships with escorts, and smaller convoys (3-4 ships with and without escorts). Even nailed the Aqautania. (Von Dos makes that ship, along with others for the game and I have it enabled). You still see the occasional big convoy as well.
The Luftwaffe will even make appearances and help you (or hinder you when they sink a ship you just torpedoed or attack when you are setting up an approach and your target starts zigging all over

) and of course, a lot of British airplanes will harass you. I have found that you have to do a lot of cruising submerged during the day, especially around Malta.
Fuel consumption has never been a problem for me in the Med. You'll run out of torpedoes a lot quicker than you will fuel.
It is definitely worth a shot. I've started alternating between the Med and the Atlantic. When I get sunk/retired in one, I start another career in the other theater. It is a nice change of pace.
