First patrol bug. Did you change/ upgrade anything on your boat before leaving base. If so, you have suffered the first patrol bug.
Never touch your boat before the first departure. Go out for a short spin, call it a shakedown, and head back to base. Then change whatever you want on the boat and go sink the enemy.
