A few things I think would be interesting (just things I'd like to see improved over SH4):
1. ASW DDs getting smarter as the game progresses
2. Magnetic detector equipped planes that fly low and help the DDs pick you up, or just better built aircraft AI and a better aircraft and carrier system in general (which would in turn open up the way for mods that allow you to play as a carrier!)
3. In-game wolf packs (ie. single player mode where you can command or take commands from other subs, and coordinate strikes on convoys)
4. Sub tenders!
5. Being able to radio for help and/or abandoning ship
6. More complicated damage and parts breakdown system (ie. electrical fires that disable parts of your ship until you fix it, shipping water if you have the hatch open while on the surface during choppy seas, etc. etc.)
7. Generally better revised ranges; historically, you could see ships out to about 15,000 yards on a good day, in SH4 your lookouts pick up stuff at shorter ranges
8. (Maybe) a sort of plot that lets you interact with characters?

9. Potential friendly fire incidents if you don't follow protocol near base, or happen to approach a friendly task force the wrong way (ie. Seawolf)
