There might be some ambiguity regarding French vessels, (Free or Vichy?) but my own pet peeve is the post-independance Indian flag... I don't know if it was changed in any of the mods but it does annoy me every time I see it in '43...
For those that don't know this is the ensign of the Royal Indian Navy to 1947:

For civilian merchants a varient of the Red Duster was used with the Star of India:
