Realism settings

When I start a new capaign how can I have custom difficulty settings ?
The only options from the drop down menu are Low, Medium, High, and Realistic. This happens even if I have custom settings selected in the Main Menu Options menu.

In bunker before patrol, hit 'Esc' to bring up realism options.

i don't think it's possible to play the tutorial (poland invasion) with custom realism settings. and if you forget to set your settings before your first real patrol, whatever you chose for the tutorial carries over.

took me a while to realize why my fuel was never being used. thought it was a bug!
