The patch is definitely from 688 Attack Sub, first sub sim I ever played! Well.... wait a minute no, I think my first was actually Silent Service on the Commodore 64. Anyway 688 Attack Sub, the box looked like a top secret government envelope and came with that patch. I had it for our family's uh..... was either a 386 or 486 computer. I think that game was actually for MS-DOS (the copy I had anyway). At the time I remember thinking the graphics were mind blowing.

They did release 688 Attack Sub for the Amiga around 1990, looks like one version for the US and one for the UK and Europe according to Mobygames.com I bought it for my IBM XT because I had CGA graphics on mine so finding software was always "interesting". I recall one of the things that attracted me was being able to fire all four tubes at once, a trick some other sims couldn't do. The ability to play both the Soviet and American sides was a nice touch too!
