Torpedo failure. This is historically accurate.
Early in the war the Germans had a lot of issues with torpedoes running too deep, exploding prematurely, not detonating at all, etc.

It's because of a built in realism feature called realistic torpedo failures which is part of .hsie realism features that most mega mods have included. If you read the v61b manual in the documentation folder within the main SH3 folder it will state the feature at what it does. Basically to ensure your torpedoes fail very rarely you need to take what ever windspeed is at the time . Ex says it 10m winds then you times windspeed by 4 divided by 10 and that's the depth you set your torpedo. So 10m x 0.4 /10 = 4 so 4m is minimum depth you need to set it.

What does this mod do, exactly ?

It does everything really, adds realism patches in, essential graphic and gameplay mods, ships etc. Basically its a modified GWX 3.0 to look better and is more realistic difficulty wise.
