In real life medics were rarely put on u-boats because they were considered a valuable resource that was better deployed on other fronts. In Iron Coffins Herbert Werner explained that on the only occassion he had a doctor aboard, the doctor was so ill with sea sickness that he was pretty much a waste of space.

In SHIII my medic has not yet been called upon, but then again I'm only on my 11th patrol in November 1940.

Most probably. The U-Tankers tended to have doctors on board, so boats on long patrols could transfer any crew members that were ill over to the tanker.
