If this were WoW, and it looks like the set up is for a rogue, he did his attack all wrong. Proper macros or timing and sequence of attack is crucial, especially for one with so low a hit point. Thus why his attack failed.
Plus, you never attack a skull rated elite alone, you're just asking for it.
