I would like to know at what realism should I play my first patrol in my career.
Are all of you start with a 100% (or very close to it) and keep it from start to the end.
Or should I just go higher and higher?

Well I would start at low realism
The event camera is good for learning what your torpedos are up to at impact - or not
The noise meter is good to start with so you get a good feel of how close you can get to shipping
Basically whatever you feel comfortable with
There are no hard and fast rules and I for one dont play 100% while many do
And many dont either - whatever is best for you but some things will help at the start

You must be experienced with navigation and calculating firing solutions. Otherwise you have no hope of sinking anything at 100%. It won't take you long to build these skills but do your first patrol with the help of your first officer. Learn from him and maybe try a few fish out without his help.

But what do you mean by the help of my First Officer? When I am in a realism fairly low and that all the computation for firing are at the basic level, does that mean that I can actually try to fire any time with the TDC and change the parameters and the system will let me do it?

Before you load the game there is a bunch of checkboxes for realism. I'm sure you know this but look for the one that say Help from TDC Officer ( something like that ).
Even if you have his help you can still go to the TDC and enter stuff manualy. You may still get too much info from him though. To learn about all this see Wazoo's Tutorial (http://www.paulwasserman.net/SHIII/).

It's Weapons Officer Help, and he is one of the only two things I leave turned on these days. The other is External View; it's just too much fun to watch all those ships and planes from different perspectives.
But they're right: start off easy and build up until you reach a level you like.
all i take away from realism is the external cam.... i like to see my boat sometimes :D
