I just installed SH3 Commander and basically I want to know, should I assign medals and promotions from the usual in-game screen, or do I do that from SH3 Commander. Commander is supposed to be more realistic, so I was wondering if that's where I'd assign rewards, or has the mod rewritten the game file that decides how many promotions/medals are assigned?
Also, in the Subsim download page are a couple of files from CaptainAmerica, and these are .7z type files. How exactly do I install these mods? are they just files I overwrite in some directory, or do I need a program to extract the files first?
I apologize if these seem like stupid questions, they seem so to me, but I'd rather get answers before doing something wrong.

You can give medals and promotions both ways. The game usually limits you to a single or small number, but in CMDR you can assign them as you wish, as long as it is legal.
7z is a type of zip file. A free program to open them is here: http://www.7-zip.org/ To install them use JSGME.

SH3Cmdr's Crew Manager component simply provides a more flexible way to reward your crew. It essentially hands the *full* responsiblity back to you. You can award via SH3Cmdr following actually historic conditions, or you can award what you want to who you want (although historically medal availability dates are modelled). It's aim is to replicate the "hmmm, I might just recommend Bernard for that EKII" feeling that is sometimes lost when using the stock and limited SH3 rewarding process.
You can award solely via SH3Cmdr. You can award solely via SH3. You can award through both. Doesn't matter. SH3Cmdr uses the same logic as SH3 when awarding medals, quals or badges, so you won't lose anything in the game by rewarding via SH3Cmdr.

what is this in the options of SH3?

fatigue model - Grey Wolves 8 Hour
and some others
if I change this...what would happen? and can I change it during an Patrol?

Those change the time it takes for the crew to get tired. I use the 24-hour model, though even with that one they get tired after 4 hours surfaced in a storm.
Yes, I have discovered that you can change them in mid-patrol with no problems.

Back with a question: Is it always safe to save a game in the middle of the patrol and leave? I ask this because I just started a new career (I love that new-career smell!), but had to leave in the middle. I sunk two ships (one of them a really quick snap shot that I'm actually quite proud of). The ships are in the captain's log as having been sunk, but anywhere it lists my total career kills/tonnage, it says I've done nothing. Is this a bug you are aware of, is there anything to be done?

If you're talking about SH3 Commander it won't list the totals until the patrol is over. I've never had a problem with mid-patrol saves.
