Far as I can tell you're not doing anything wrong! Just curious what is your current rank?
If it helps, I'm currently Oberleutnant zur See (Lieutenant Sr) and my career stats as follows after my 32nd patrol:
Patrols - 32
Renown - 15,771
Start Date - 9/1/1939
End Date - 10/11/1942
Tonnage - 776,986
Medals - All of 'em, case is full!
Yet I still haven't reached Kapitänleutnant rank.
I don't know, but I imagine promotions are partly based on the realism setting. I'm playing on about 55% (my math skills just aren't up to doing manual plots :( )
Here's a link giving a little info on the time needed to reach Kapitänleutnant in the real Kriegsmarine:

I believe that you need 25,000 renown to be promoted to Kapitänleutnant. That is 25,000 in stock not your career total so stop wasting it on trivial things like radar detectors, anti sonar coatings, bigger boats, snorkels etc.

I dont think so, I'm a Kapt Lt now and I'm only on my 9th patrol with something like 16500 renown and about 450000 tons. The criteria for rank must be linked to something strange. I am using the RUB mod, it probably has something to do with this.
From the basic cfg file.

With 776K Tonnage shouldn't you be a LT Commander by now?

Well I should hope so! Maybe you can put in a good word for me with bdu? ;) They won't listen to a thing I say!
I'm gonna take the advice here that you need 25k renown, after the patrol I posted, I had transferred to flotilla 2 and bought a nice new IXC with all the options leaving me with 5k renown. So I went back to my last save with 15k renown, only got 10k to go. Shouldn't take but a few patrols :)

Just got promoted to Kapitänleutnant :) On my 33rd patrol, and with only 18,042 renown so I don't think that 25k renown is totally valid, there must be other factors that come into play.
Was a good patrol, my best yet:
4 crew promotions
4 IC 2nd class
2 IC 1st class
2 German Gold Cross
1 Knight's cross
Ships and tonnage:
C3 - 7,907
Sm Merch - 2,428
Costal Merch - 2,016
C2 - 6,446
C3 - 7,950
T3 - 11,673
C2 - 6,448
T3 - 11,676
T3 - 11,675
T3 - 11,716
C3 - 7,952
Total tonnage - 87,889

In RUb the requirements for promotion have been reduced by about 66% across the board, mostly because the game starts you off too low in rank compared to real commanders (hardly anyone commanded a U-boat as a Lt. z. See). Also, it was reduced because it was virtually impossible to get to KptLt unless you starved yourself of upgrades and got a huge score.
