Running win xp, want to run @ 1920-1080, full screen.

Are you running any mods? SH4 was written for Windows XP, so you shouldn't have issues setting a resolution that matches your equipment. That is the key point - matching your equipment. The game run best when you match resolution throughout the chain. All monitors have a "Native" or "recommended" resolution. Most old 17" CRT monitors had a "Native" resolution of 1280 x 1024 4:3 at 60-70Hz, depending upon model. They could be driven at any resolution below that, but it was recommended not to drive them any higher than that. LCD monitors usually are seen with a "recommended" resolution in Windows, and you should match that also. So, if you have a 16:9 monitor, which is what 1920 x 1080 is, then you should match that in Windows. Whatever you set Windows to, should show in the SH4 game's Options menu, Graphic Settings screen. btw, the list of resolutions available in the SH4 Menu is NOT listed in order, with some of the newer resolutions being near the bottom of the list.

you should not have a problem with that, in fact, running SH4 in full screen is recommended.
on the main menu screen, click options, then graphics settings. i've included a screen shot of how you should set your options. (your refresh rate should match Windows which should match your monitor)

when you finish changing your settings, exit SH4 and re-enter and voila, the changes will have taken hold.

also, do not shell out to Windows while you are playing SH4. Shell out = Windows key at bottom of keyboard or Alt-tabbing to work with another app. SH4 does not like it when you (or anyone else) does that and strange things happen including but not limited to sudden and immediate crash-to-desktop (aka CTD).
