Ship journal multiple entries

So I started using the ship journal. I like using it. Problem, when I open the ship journal to view it when not in game (I open it at menu to view and the journal entries show up) I see multiple same entries. It seems every time I open my ship journal in game, and use it, then save it after a game save... it reposts all ship journal entries again... basically copying them again.

What is the proper procedure to open the journal, use it, and close it without creating multiple copied entries? Thanks in advance.

Also how to open the journal viewer with out starting the game menu?
