Is there ONE mod that is a must have?

I don't want to pick and choose, if I can avoid it. I'm just wondering if there is one mod for SH4 that is kind of a catch all/fix all? I keep reading about TMO, so is that a good one to get?

There's actually several. TMO, RFB, FOTRS, and GFO are all meant to be all-in-one mods, but they include options within themselves.

TMO is a good one, especially if you're looking for something high-difficulty! The others are good options as well. GFO (Game Fixes Only) is a good one if you're just starting out, and you're looking for something that isn't too drastic of a gameplay change from the out-of-the-box SH4.

Mod's come down to personal tastes... There's no definite answer to give, some like apples others oranges yet others like peaches.
My advice is, try them out one at a time for a couple of patrols and decide for yourself what you like.

Once you have a bases you can start building on it, like Env., Eye candy and other attributes.

CCIP is right on maybe go with a good sound mod if you do manual targeting otc mod the way to go and rsrd on top of a mod you choise

If you can do multiple installs I would recommend you try some or all of the 'super' mods mentioned here.

As stated, it's all a matter of personal preference as each mod brings it's own flavour to the game.

I use TMO and RSRD.
TMO adjusts/improves certain aspects of actual gameplay and graphics. The intent being to make things more difficult and realistic (i.e. more challenging).
RSRD is essentially a historical campaign "layer" which attempts to make your campaign etc more historically accurate.

If you read up on TMO and RFB you'll understand some of the differences. Again, if you can do it, go with multiple installs try the mods seperately for comparison.
