Yes, software is always perfect. I've never had a program just stop working before.

Are you forgetting the Progress that smacked in to Mir? :03:That 43 of 44 is referring to ISS flights only. Total flight count is (if I have not missed any) 129 of which 2 have failed. This means success rate of about 99,9845 %. This ofcourse assuming that I have not missed any crashes.
