missing from options/graphics

the last settings in both colunms aren't there. i know the left is v-sync, but i don't recall what the right is supposed to be. all that's there for both is the check box

Your game screen reoloution is probably bigger than your desktop resoloution and you probably have the game set to run in wondowed mode.

game res and screen are the same, 1280*1024 and windowed mode is the 2nd from top in left column

In that case the only other thing I can think of is something called "composite sync".
I have an LCD monitor and I did have an issue with some programs being off center. I use an ATI PCIexpress card and there is a box I checked in the ATI control panel under monitor/view and that did it for me. Also I read somewhere that an LCD monitor (19 in) is happiest at 1280x960. If your equipment is different, your milage may vary :)
