Have you tried changing synchronized to yes under [video] in main.cfg (in your documents\sh4\cfg\ folder)? That did the trick for me as far as vsync.
Thanks DJSatane for pointing out that we need to manually change the VSync for LCDs. It made a notable difference with mine. I didn't realize it was off by default and perhaps no way to change it in-game.:rock:
