Optional medical insurance has the unfortunate consequence of making things more expensive.
If it's optional, then healthy & confident people won't bother, most insurance will be bought by moderate-high risk people. Premiums rise due to more claims, leading to fewer low-risk people buying insurance. This continues for a while. Mandatory insurance could come out, on average, to be a better deal. But that depends on thinking on a national level, which voters hate to do.
