Default Looking for a layout when writing in a notebook.

I'm pretty new to Silent hunter series. Grabbed 5 and the WoS mod and am having fun learning, even though I have yet to manage to sink anything without having to reload a half dozen times. Running a 100% realism campaign so I pretty much dove into the game head first.

Anyways, I have a composition book that I want to use as my "log" when running the TDC but I don't know a good layout. I think this is the main reason I'm having trouble because, at the moment, I am just jotting stuff down on the page as I take readings in no particular order so I cannot, quickly and efficiently, get the data implemented into my TDC.

I was thinking that the real Kriegsmarine or US Navy had a book specifically for contacts that had things laid out in a specific order. Something like:

Contact Type / Bearing (Time) / Distance (Time) / Speed (Time)

Better yet, if there was a mod with a realistic notebook with this sort of information that you could fill out, that'd be pretty cool too so that I can, easily, see at a glance all the info I have on a contact.
