Saved games are notoriously twitchy in SH3. Try this thread for more information:

I got less CTDs once I applied the 4GB patch but the best advice is to not rely on save games and to never, ever save within render distance of land, ships or aircraft - I usually let myself get at least 50km from both land and other known ships before I try and save mid patrol.
