The documentary itself is pretty accurate the Ustinov report made for very blunt and chilling reading at the time.

What is known is the 65-76A was responsible, this type of torpedo had been discarded by the west back in the 1960's and has been known to sink submarines.

To say they had poor maintenance would be an understatement I think, a decade of complete under funding if any at all, plus the rife pilfering was just a disaster waiting to happen.
