Accidental torpedo launches?

I was wondering if there are any recorded cases of WWII subs accidentally launching torpedoes, and what the cause of the accident was. Cheers :Kaleun_Salute:

Would make for a very difficult accident. The bow caps and muzzle doors would need to be open and if memory serves they were opened by hand in the torpedo room at that time. I would think that by that time there would be interlocks to prevent such a thing.

Not a submarine but USS William D Porter makes for a good read her unfortunate issues shall we say did once culminate in the accidental firing of a torpedo at the battleship USS Iowa while President Roosevelt was on board.
